Clinical notes record what happened during a patient visit. In OdontoX, you can type notes, dictate them, or use AI to restructure free text into SOAP format. Who can do this: Admins and Doctors.

Where to write notes

You can write clinical notes in two places:
  1. Appointment detail page → Overview tab — the main note block for the current visit.
  2. Dental chart → per-tooth panel → Notes tab — notes tied to a specific tooth.

Write or dictate a note

  1. Open the note editor.
  2. Type directly, or click the microphone to dictate.
  3. The recorder supports English, Urdu, and Hindi.

Use AI tools

The toolbar below the editor offers:
  • Rewrite — rephrase for clarity
  • Grammar — fix grammatical issues
  • Shorten — condense verbose notes
  • Detailed — expand brief notes
  • Structure with AI / AI SOAP — restructure free text into SOAP format

SOAP format

SOAP stands for:
  • Subjective — what the patient reports
  • Objective — what you observe or measure
  • Assessment — your diagnosis or clinical impression
  • Plan — treatment and next steps
Click AI SOAP to convert your dictated or typed note into this structure.

Auto-save

Notes auto-save shortly after you stop editing. You can also click Save explicitly.
